
Dr. Jose Azar MD
Chief Quality Officer, IU Health; Co-Founder/Chief Engagement Officer, CHIIS, Indiana University Center for Health Innovation and Implementation Science
Dr. Azar is a founding member of the Indiana University Center for Healthcare Innovation and Implementation Science (IU-CHIIS) where he works to discover and rapidly implement innovative healthcare solutions in the real world. This effort intends to transform the healthcare system into a highly reliable learning system that can accomplish the triple aim of “better health, better care at lower cost”. Through the CHIIS, he serves as the Chief Engagement Officer for the Great Lakes Practice Transformation Network (GLPTN) which is funded by CMS with 50 Million USD for 5 years and has enrolled 15,000 physicians in transforming their outpatient practice in preparation for payment reform. Dr. Azar is also a faculty member of the Certificate of Innovation and Implementation Science provided by the CHIIS in order to train and prepare healthcare leaders in creating a learning healthcare system.
Dr. Azar's work with the CHIIS complements his other role as Chief Quality Officer for Indiana University Health where he implements his vision of creating a learning healthcare system and working to transform the organizational culture into one that continuously works at reducing suffering from the Healthcare delivery system. Beyond Hospital Acquired Infections and overall safety, he has extended his scope to address other areas of unintentional harm such as emotional and psychological harm that patients experience.
