Short hospital stays, higher patient acuity and limited access to ostomy specialist, have challenged the education of patients discharged from the hospital with new ostomies. This places an additional burden on the home health nurse in preparing patients for independence in living with an ostomy. A consensus conference of certified ostomy nurses, hosted by the Wound Ostomy and Continence Nurses Society, developed eighteen statements to guide clinical practice in preparing patients for discharge from home health. This session will present the outcome of the consensus panel, and take a look at the supportive discussions and implementation strategies.
