Jay R Horton ACHPN, FNP-BC, MPH

Jay R. Horton, ACHPN, FNP-BC, MPH is the Clinical Program Manager of the Interdisciplinary Palliative Care Team at the Mount Sinai Medical Center in New York City. He serves on the board of the Hospice and Palliative Care Association of New York State where he co-chairs the Palliative Care Advisory Group. Mr. Horton is also a State Public Policy Ambassador from New York for the Hospice and Palliative Nurses Association. He co-developed the curriculum for the Palliative and End-of-Life Care Subspecialty Program as Assistant Professor at the Columbia University School of Nursing, where he continues as Adjunct Professor. Mr. Horton received his MSN from the Yale School of Nursing and his MPH in health policy and administration from the Yale School of Medicine, Department of Epidemiology and Public Health. He completed fellowship training in Pain and Palliative Care at Memorial Sloan-Kettering Cancer Center and is currently pursuing a PhD in Nursing at the NYU College of Nursing, where he is conducting palliative care outcomes research.
