Program Overview:
Worldwide, wounds afflict between 6 and 22% of patients in the acute care setting, affecting 2.5 billion people a year. Presenter Teresa Novy found that, at her facility at least, many of these patients were being released unprepared. This course discusses one facility’s efforts to improve patient outcomes by implementing standardized electronic discharge instructions and wound care orders during transition from acute care to home.

This bite sized learning sessions explores how Teresa’s facility implemented an adapted, standardized, electronic version of the Project RED discharge wound care order set. This was done as a way to improve communication about discharge wound care orders between physicians, wound care nurses, and home health nurses.

Learning Outcome:
Upon completion of this program, participants will identify one way to provide a more patient-centered discharge and teaching plan.
