Error disclosure is a challenging interaction for patients and families as well as clinicians. When done well, they can lead to meaningful resolution after a harm event. When done poorly, they can make a bad situation worse. In this session, nursing, physician and risk management leaders will guide learners through the process of error disclosure, step-by-step. Learners will be provided a framework on how to conduct these critical conversations with patients and families that includes a focus on the initial communication and the aspects of necessary on-going follow up. As a result, learners will feel empowered to conduct robust disclosure conversations.



Objectives:

Prevention of the diabetic foot is now being advocated by different healthcare specialties of medicine, other than just the foot doctors, more than ever now. management of These other areas are now calling out for a multidisciplinary approach for the management of diabetic foot ulcers. Prevention using the educational method is also increasing in interventions being used.

  • Learners will advocate and increase communication about the need for more public awareness to assist diabetics to improve their self-management skills.

Bladder control challenges can impact a person’s dignity and quality of life at any age although risk factors increase as one ages. This session will address pharmacological management of bedwetting and lower urinary tract symptoms of urgency, frequency, urge incontinence, and nocturia across the lifespan utilizing case studies to assist the WOC nurse specialist to apply theory into everyday practice.

Gap: Many Wound Ostomy Continence nurse specialists do not have the knowledge to identify factors promoting bladder control across the lifespan and implement appropriate management strategies. Identifying the lower urinary tract symptoms and selecting medications to manage these urinary dysfunctions needs to be reinforced.



Objectives:
  • The learner will be able to transfer knowledge into practice of pharmacologic concepts to the management of individuals throughout their lifespan with bladder control issues such as nocturnal enuresis, overactive bladder, and nocturia.

Skin infection usually requires prompt attention depending on severity and patient context. If antibiotics are needed, they must be utilized with reflective thought. Does this infection need systemic antibiotics? Could this infection be prevented? This session will focus on prevention, recognition, and management of wound/skin infection incorporating antimicrobial stewardship.

  • Learners will be able to discuss approaches to wound/skin infection prevention, recognition, and management and appropriate antibiotic usage.

This session will address several ostomy related challenges: parastomal hernia prevention; colostomy irrigation; stoma site marking; difficult pouching situations; and counseling regarding sexuality and intimacy. The current evidence will be presented as well as approaches to management of these clinical challenges.

  • Participants will use current evidence to manage ostomy related challenges, including parastomal hernia prevention, colostomy irrigation, difficult stoma site marking, counseling re: sexuality and intimacy, and difficult pouching situations.

Home care is an integral part of the health care system. Current challenges include short staffing, burdensome documentation requirements, patients with a high acuity and diminished resources to address their care needs. This session will address the role the WOC nurse plays in influencing and managing home care patients with wound ostomy and continence needs applying current resources, wholistic care, and pragmatic application of cost containment.

  • State the strategies used to manage patients with wound ostomy and continence issues in the home care setting while maintaining quality and cost awareness.

It is known in the literature about the clinical and economic benefits of instillation therapy, ranging from reductions in hospital stays to fewer OR visits and beyond. This symposium will take a deeper look at the latest published data, new therapy indications and clinical applications of the newest innovations in NPWT - Instillation Therapy.

Speakers will share cases that demonstrate the utility of and advances in Instillation Therapy when tackling wounds from head to toe. They will share practical techniques that help simplify its application. Panelists will stimulate out-of-the box thinking during the Q&A session.

The prospect for, and experience of, having ostomy surgery can be associated with feelings of hopelessness and helplessness. Mental health challenges, issues with altered sexual health, and changed body image can occur without intervention. This session will target strategies to assist with ostomy creation adaptation across the lifespan including reflection of clinicians on whether they believe high quality of life is possible.



Objectives:
  • Learners will analyze mental health issues associated with ostomy surgery across the lifespan and possible related adaptive strategies including clinician self-reflection on personal beliefs.
